Grievance

This form is to be filled out if you wish to file an official complaint against another Guilde Member or Officer. The Grievance process is very serious, and a Grievance should never be submitted jokingly. Download

This form is to be filled out if you sustain an injury during a Guilde event. Download

This form is to be filled out at the end of a training session and is to be turned into the Training Coordinator. Download
